Public Records Request

Request is hereby made for the following public records (please indicate document names, page numbers, addresses, and/or permit numbers where applicable):

A "commercial purpose" is defined by A.R.S. §39-121.03 as the use of public records for:

  • Sale or resale, or to produce a document containing all or part of the record (such as a copy, printout, or photograph) for sale, or
  • Obtaining names and addresses from such public records for solicitation, or
  • For a purpose in which the purchaser can reasonably anticipate monetary gain from the direct or indirect use of such public record.

The following applies to all public records requests:

  1. After receipt of your request, you will be notified when the public records are available for inspection or pick-up. Payment is due for copies when they are picked up.
  2. If paying by check, please make the check payable to the Town of Pinetop-Lakeside.
  3. Charges:
    • Police Department:
      • Police Reports... $10.00 for documents that are 10 pages or less; $1.00 for each page over 10
      • Video... DVD - $20.00 per disc
      • Audio... CD - $20.00 per disc
    • Non-Commercial Requests:
      • E-mailed documents: No charge.
      • Paper copies: $0.50 per page (letter size, 8" x 10"), or $1.00 per page (legal size, 11" x 17")
      • CDs, DVDs: $5.00 per disc copied. (Please note that there is no charge for simply locating the requested documents; you can view or listen to them at Town Hall for free.)
      • Photographs:
        • 3" x 5" color copy - $5.00 per copy
        • 8" x 10" color copy - $10.00 per copy
        • CD of digital photographs - $5 per disc
      • Individual departments should be consulted for fees associated with special formats such as oversized prints, color copies, etc.
    • Commercial Requests:
      • The charge for commercial public records requests will be based upon:
        • The value of the reproduction on the commercial market as best determined by the public body,
        • A reasonable fee for the cost of time, materials, equipment and personnel in producing such a reproduction,
        • And a portion of the cost to the public body for obtaining the original or copies of the documents, printouts or photographs. (See A.R.S. § 39-121.03(D))
    • Custom Requests:
      • A custom request is one for a record or format that does not currently exist. If a custom request is made, and the Town determines that it can fulfill the request, we will contact you with the estimated cost first.
